Adoption Application Form

Fallen in love with a Dali Dog and ready to adopt? Complete this form and a member of our team will get back to you to progress your application.

Important Information

  • Before completing this form, please ensure you have read and understood the Terms and Conditions below.

Adopting a dog is a long-term commitment (10-18 years in the case of a puppy); please ensure you can take of the care of the dog, both practically and financially, before deciding to submit your application.

  • COVID-19 - reducing risk during home checks: could you please take a short video of your home and garden, (or take photographs if you are unable to video), and return with this form. We also need a photo of ID (passport or driving license) that the home checker will use on arrival to confirm they have the right home/person.

  • You should expect to pay £660 for your dog (£590 for puppies). This cost includes the adoption fee, transport costs and Traces.

The adoption fee (see below) will be requested on completion of your Adoption Application form to reserve the dog. If you change your mind, this is not refundable. If your home check is not approved then the full amount will be refunded.

Adoption Costs: For dogs aged between 8-12 months and over, we ask for a minimum fee of £225, to contribute towards the preparation of your dog for travel to the UK. This includes spaying/neutering; Snap 4Dx which tests for exposure to pathogens that cause Heartworm disease; tests for Ehrlichiosis, Lyme disease and Anaplasmosis; and Leishmania snap test. It also includes dog/puppy vaccines, rabies vaccine, microchip, worming treatment, tick and flea treatment, fit-to-fly examination and an EU-pet passport.

For dogs aged under 8-12 months, the same preparation takes place (minus neutering/spaying) and the minimum fee is £155. There will be a requirement to have the dog spayed/neutered by your own vet, when it reaches the age recommended by your vet for doing so, at your expense.

Travel Costs: In addition to the Adoption Costs above, you are responsible for paying to us the flight and onward transport costs of £ 435.00. Most of our dogs fly from Cyprus to mainland Europe and are met there by our Pet Couriers who arrange a collection point with you. Also included in the total travel costs are traces and the cost of returning your dog’s travel crate to Cyprus. Traces are a requirement of the UK government and must be completed before any animal is imported into the UK.

    When to pay:

A payment of £225.00 will be requested to reserve your dog (£155 for puppies). The remaining payment will be requested after the notification of a successful home check. Payments can be sent via PayPal (friends and family option) to dali.rescue@hotmail.com (please ensure you state your dog’s name in the notes).

    Gift aid
If you are a UK resident and you choose to donate to us regularly following your adoption, you can add gift aid and we’ll get an extra 25p for every £1 you donate. Click here to complete our gift aid form.

  • Please note that the courier vans will deliver to Berwick Upon Tweed, England or Todhills, England. You must be prepared to travel to one of these destinations to collect your dog.

Adoption Terms and Conditions

Adoption Terms and Conditions

Application: Please be advised that completing the Adoption Application Form is not a guarantee of adoption and D O G Rescue Cyprus reserves the right to refuse any adoption that we feel will not suit the dog’s requirements.

Adopter: Adopters must be a minimum of 21 years of age. For senior adopters we may proceed on the understanding that a family member will take the dog if you become unable to care for it.

Home check: We aim to have your home check carried out by one of our volunteers as soon as possible after receiving your Adoption Application Form - ideally within 2 weeks.

Health: D.O.G Rescue Cyprus recommend that your dog is registered with a vet and visits them within 5 days of being homed for a health checkup. All dogs are treated for parasitic infection before departure from Cyprus, but we recommend you take advice from your vet regarding treatment on arrival, as we cannot guarantee that the treatment administered upon departure will completely eradicate any parasitic infection.

All our dogs are blood tested for Ehrlichiosis, Lyme disease, Anaplasmosis and Leishmania (diseases common in Mediterranean countries) prior to travel. However in a small number of cases it is possible for a disease to remain dormant for several years with no clinical signs or symptoms. For example, stress factors can cause Leishmania to become active but dogs who are quickly diagnosed can be treated effectively and live a happy life, with daily medication. We advise to test for Leishmania YEARLY with an ELISA or iFat test. We also request that should your dog become ill that you contact us. We are always here to support you and our team and our Cyprus vets can often recognise the signs and symptoms of Mediterranean diseases much quicker than some UK vets as they have more experience of them. This can help a quick diagnosis and speedy recovery.

D.O.G Rescue Cyprus does everything reasonably practicable to ensure the health of your dog. However, as a living being we are unable to guarantee the health of your dog. If your animal becomes ill, you must be financially and emotionally prepared to treat your dog at your expense.

Neuter/Spay: Most of our rescue dogs are neutered/spayed before flying, but on the occasion that they are too young for surgery, it is a compulsory condition of your adoption that at your expense you commit to neuter or spay your puppy once they are old enough and provide a vets written confirmation that this has been undertaken. Failure to comply may result in Dog Cyprus removing the dog from your care. This will be part of our follow up checks. Puppies under the age of 8-12 months and subject to development, may not be neutered/spayed before travel, and we will advise on an individual basis.

Follow up: DOG Rescue Cyprus will stay in touch with you, and retain the right to also carry out a further visit to your home after the dog is placed, as a follow-up check. We request that you keep us updated with any change to your contact details, including a change of address. Dali Dog Rescue UK have the right to remove the dog from your care if your circumstances have changed to the degree that they no longer meet our rehoming criteria and, as such, would fail any future follow up home checks.

D.O.G Rescue Cyprus CANNOT guarantee either the age, or the breed (or

breeds), of a dog. All our dogs are microchipped prior to travel, and are registered under our charity with PETtrac, for their safety at the point of entry to the UK. It is a legal requirement that you update the registration with your own details, and keep it up-to-date, by completing our Microchip Registration form and returning it to us by following the instruction on the form available on our website. The cost is £20.00 and form and payment must be submitted within 14 days of arrival. Failure to comply may result in Dog Cyprus removing the dog from your care.

Please ensure you have your dog fully covered by pet insurance. Should you choose not to take out pet insurance, it is a condition of adoption, that any future medical needs will not be restricted in any way due to personal finances. Failure to comply may result in Dog Cyprus removing the dog from your care.

We want your dog to settle in to your home, but some do take longer than others, so please be patient. We do not want dogs returned within a few days of adoption. You will not see the true character for possibly weeks to several months. Your dog needs time to get to know you and your home.

The Adopter agrees to comply with all relevant regulations, legislation and laws concerning the welfare, control, responsible ownership and protection of animals including without limitation the Animal Welfare Act 2006 and Codes of Practice issued in England and Wales under the 2006 Act and to ensure that anyone with the temporary care of the animal also complies. This means providing for the animal’s health and wellbeing, and taking measures to ensure that the animal’s health is maintained, he/she is properly housed, fed, watered, socialised and exercised and receives appropriate veterinary care, including vaccination, regular worming and flea treatment.

The Adopter accepts that D.O.G Rescue Cyprus makes no representation or warranties, expressed or implied, about the dog’s temperament and, to the fullest extent permitted by law, accepts no liability for any loss or damage caused by the dog.

D.O.G Rescue Cyprus MUST be informed at any time of a change in circumstances and the dog returned to one of our volunteers where required. Our dog must never be handed over to any other UK Rescue. We always accept the return of our dogs and will endeavour to rehome them accordingly.
